Home Health Licensed Practical Nurse - Full Time

Position Overview

The Licensed Practical Nurse provides skilled nursing care by following care plans and physicians' orders, ensuring confidentiality and professional boundaries.

Key Responsibilities

• Deliver quality patient care under the supervision of a Registered Nurse.
• Perform hands-on client care, document activities, and communicate with physicians and caregivers.
• Recognize and respond to changes in patient needs to prevent or reduce risks.
• Educate clients and families based on individual needs.
• Provide home-based care using skills such as assessment, wound care, and phone triage.

Qualifications & Skills

• Active LPN license in the applicable state.
• Valid CPR certification.
• Preferred experience in Medicare Skilled Nursing, home health, or healthcare settings.
• Knowledge of Oasis and Home Care Home Base is a plus.
