Home Health Licensed Practical Nurse - Full Time

Position Overview The Licensed Practical Nurse provides skilled nursing care by following care plans and physicians' orders, ensuring confidentiality and professional boundaries. Key Responsibilities Deliver quality patient care under the supervision of a Registered Nurse, including documentation and communication with physicians and caregivers.Recognize patient needs and responses, implementing interventions to reduce risk.Educate clients and families based on individual needs.Perform in-home care tasks such as wound care, infusions, catheter care, and PICC line dressing changes. Qualifications & Skills Active LPN license in the applicable state.Valid CPR, driver's license, and proof of automobile insurance.Preferred: 1-year LPN experience in home health or healthcare settings; Home Care Home Base experience is a plus.
