Home Health Licensed Practical Nurse - Full Time

Position Overview The Licensed Practical Nurse provides skilled nursing care, documents patient progress, and follows physicians’ orders while maintaining confidentiality and professional boundaries. Key Responsibilities Deliver quality patient care under the supervision of a Registered Nurse.Perform hands-on care, document activities, and communicate with physicians and caregivers.Recognize and respond to changes in patient needs to prevent or reduce risks.Educate patients and families based on individual needs.Provide in-home care using skills such as wound care, infusions, catheter care, and dressing changes. Qualifications & Skills Active LPN license in the applicable state.Valid CPR certification.Preferred: Medicare Skilled Nursing experience, basic Oasis understanding, 1-year home health or healthcare experience, and Home Care Home Base knowledge.
