Home Health Licensed Practical Nurse - Full Time

Role Overview

The Licensed Practical Nurse provides skilled nursing care in patients' homes, following care plans and physicians’ orders while maintaining confidentiality and professional boundaries.

Responsibilities

• Deliver quality care under the supervision of a Registered Nurse.
• Perform hands-on client care, document activities, and communicate with physicians and caregivers.
• Recognize and respond to changes in patient needs to prevent or reduce risks.
• Educate patients and families based on individual needs.
• Provide diverse home care skills such as wound care, infusions, catheter care, and dressing changes.

Qualifications & Skills

• Active LPN license in the applicable state.
• Valid CPR, driver’s license, and proof of auto insurance.

Preferred Experience

• Medicare Skilled Nursing experience.
• Basic understanding of Oasis and home health software.
• One year of LPN experience in home health or healthcare setting.
