Home Health Licensed Practical Nurse - Full Time

We are seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes, supporting recovery and independence. Key responsibilities include administering treatments such as wound care, infusion, catheter care, and PICC line dressing changes,maintaining accurate clinical documentation in compliance with regulations,coordinating comprehensive patient care,consulting with physicians to adjust care plans,communicating respectfully with clients, families, and team members, andobtaining and updating patients' medical histories. Qualifications include an active LPN license in the applicable state,valid CPR certification, and a valid driver’s license. Preferred experience includes at least one year in a healthcare setting.
