Home Care CNA - Weekends Only

About the role

Certified Nurse Assistant (CNA) providing in-home care primarily on weekends.

Responsibilities

• Providing direct patient care, including personal hygiene, transfers, ambulation, and basic nursing care.
• Assisting with prescribed exercises and meal preparation, considering dietary needs.
• Offering companionship and engaging in activities tailored to individual needs.
• Performing light housekeeping duties and maintaining accurate records of care provided.
• Monitoring and reporting changes in patient's physical or psychosocial condition.
• Following the care plan strictly, including documentation via EVV app or time sheet.
• Upholding patient confidentiality, privacy, and rights, and reporting concerns such as abuse or neglect.
• Using equipment like blood pressure cuffs, stethoscopes, and transportation as needed.

Qualifications

• Current certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills and physical ability to perform tasks such as lifting and bending.
• Compassionate attitude toward patient care.

Key duties

Deliver compassionate home care within clients' residences, adhering to the care plan, and ensuring high-quality service. Must be dependable, adaptable, and respectful of client and family values.
