About the role
Certified Nurse Assistant (CNA) providing home care services to patients in their residences.
Responsibilities
- Delivering direct patient care, including personal hygiene, mobility assistance, and basic nursing tasks as instructed by healthcare professionals.
- Assisting with transfers, repositioning, and ambulation to promote patient comfort and safety.
- Preparing meals aligned with client preferences and dietary requirements.
- Providing companionship and engaging in activities tailored to individual needs.
- Performing light housekeeping and maintaining accurate records of care provided.
- Monitoring and reporting changes in the patient's physical or psychosocial condition.
- Following the care plan strictly and documenting tasks via EVV system.
- Ensuring confidentiality, respecting patient rights, and reporting any abuse or unsafe conditions immediately.
Qualifications
- Current certification with DFS.
- High school diploma or GED preferred.
- Effective communication skills and ability to follow directions.
- Physical ability to lift approximately 50 lbs, bend, and stand for extended periods.
- Compassionate attitude toward patient care.
Skills
- Providing quality, compassionate care, adhering to the care plan, documenting accurately, and demonstrating dependability and adaptability in a home environment.
- Physical tasks and travel within the assigned area are essential components of the role.
