About the role

BayCare Home Health is seeking clinical Team Members passionate about providing outstanding patient care and customer service. Responsibilities

• Provide compassionate and professional care to patients in their homes.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Document patient progress and communicate effectively with healthcare providers.
• Adhere to infection control protocols and maintain a safe working environment.

Requirements

• Licensed Practical Nurse (LPN) with BLS certification.
• Licensed Practical Nursing (LPN) license.
• Driver's License from the State of Florida.
• RN with BLS certification.
• Registered Nurse (RN) license.
• Driver's License from the State of Florida.
• Physical Therapist with BLS certification.
• Physical Therapist license.
• Driver's License from the State of Florida.

Schedule

Full-time positions available, Monday through Friday, 8:00 AM - 5:00 PM.
