About the role

BayCare Home Health is seeking clinical Team Members for various roles including LPN's, Home Health Clinicians, Case Managers (RN's), Physical Therapists, and Physical Therapist Assistants. Positions are full-time, Monday through Friday from 8:00 AM to 5:00 PM, available in Hillsborough, Pinellas, Pasco, Polk, Manatee, and Sarasota Counties.

Responsibilities

• Provide compassionate and professional patient care
• Collaborate with interdisciplinary teams to develop and implement care plans
• Document patient progress and communicate effectively with patients, families, and other healthcare providers
• Adhere to infection control protocols and maintain a clean, safe environment
• Perform physical assessments and administer treatments as prescribed

Requirements

• Licensed Practical Nurse (LPN) with BLS certification
• Licensed Practical Nursing (LPN) License
• Driver's License, valid in Florida
• RN with BLS certification
• Registered Nurse (RN) License
• Driver's License, valid in Florida
• Physical Therapist with BLS certification
• Physical Therapist License
• Driver's License, valid in Florida
