Healthcare Provider - Nurse Practitioner or Physician Assistant

About the Role

Provide primary and consultative care to geriatric and medically complex patients, often in their homes or care facilities. Conduct comprehensive evaluations, develop treatment plans, prescribe medications, and collaborate with physicians and caregivers to improve patient outcomes and quality of life.

Responsibilities

• Deliver patient-centered, high-quality care focusing on comfort, function, and connection
• Coordinate closely with interdisciplinary teams and families
• Manage chronic illnesses, prevent hospitalizations, and support patient education
• Travel within designated service areas to provide in-home or facility-based care

Requirements

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
• Current state licensure and reliable transportation

Qualifications

• Preferred: 2+ years clinical experience, gerontology certification, or experience with geriatric patients
• Strong nursing skills, ability to assess patient needs, handle multiple priorities, and communicate effectively

Pay

Competitive salary ($110,000–$125,000) with monthly bonuses.
