The Role
The Geriatric Case Manager serves as a trusted guide for clients and families—assessing needs, coordinating care, and ensuring services are delivered with excellence and compassion. This role blends clinical judgment, problem-solving, and relationship management in a fast-paced, meaningful environment.
Key Responsibilities

Conduct comprehensive in-home assessments and ongoing reassessments

Develop and update individualized care plans

Serve as a primary point of contact for clients and families

Provide education, guidance, and emotional support during transitions of care

Collaborate with caregivers, nurses, physicians, hospice, and community providers

Monitor quality of care and intervene proactively when issues arise

Coordinate services such as medication support, safety planning, and disease-specific care

Support caregivers with care guidance and client-specific education
-
Communicate changes in client condition to internal teams

Participate in care conferences and interdisciplinary collaboration

Maintain accurate, timely documentation

Ensure services align with state regulations and company standards

Support quality assurance and outcome tracking efforts
Qualifications
-
Bachelor’s degree in Social Work, Nursing, Gerontology, or related field

Minimum 3–5 years of experience working with older adults
-
Strong knowledge of aging, chronic disease, dementia, and care transitions

Excellent communication, organization, and problem-solving skills
-
Reliable transportation and ability to travel locally

RN, LCSW, LMSW, or Care Manager Certification (CMC) (preferred)

Experience in home care, home health, hospice, or geriatric care management (preferred)

Familiarity with community resources and healthcare systems (preferred)
