Full Time LPN Medical Team Administrator

About the role

Southern Health Partners (SHP) is seeking a dedicated Licensed Practical Nurse (LPN) Medical Team Administrator (MTA) to support our correctional healthcare team at Wood County Jail in Wisconsin. This is an opportunity to combine hands-on nursing knowledge with clinical coordination, team support, and daily healthcare operations. You'll help keep care organized, support your fellow healthcare professionals, and ensure patients receive consistent, compassionate treatment in a safe and secure correctional setting. Registered Nurses (RNs) are also encouraged to apply.

Responsibilities

• Coordinate daily healthcare operations and help ensure quality patient care.
• Provide nursing assessments, treatments, medication administration, and other clinical services within your scope of practice.
• Support and coordinate healthcare team activities to promote efficient, consistent care.
• Assist with patient care planning, chronic disease management, and follow-up services.
• Maintain accurate medical records and ensure documentation meets established standards.
• Communicate with healthcare providers, facility staff, and members of the medical team regarding patient needs.
• Help maintain compliance with applicable healthcare regulations, policies, and clinical protocols.
• Participate in on-call responsibilities to help address healthcare needs outside regular scheduled hours.
• Promote a professional, respectful, and patient-centered healthcare environment.

Schedule

Monday–Friday, 6:00 AM–2:30 PM with on-call responsibilities.
