Full-Time Clinical Nurse - Transitional Care Unit

About the role

Provides direct patient care activities, including assessment, diagnosis, planning, implementation, and evaluation within nursing standards. Collaborates with a multidisciplinary team to deliver exceptional care to a diverse patient population with complex medical needs. Opportunity to gain experience in a Level I Trauma Center and prepare for specialty areas like the Emergency Department. Responsibilities

• Promotes patient and family-centered care, educating on illness management and post-treatment needs.
• Participates in safety, quality, and regulatory activities, and engages in professional development.
• Develops, implements, and documents personalized care plans with clear goals, ensuring effective communication across care teams.
• Uses critical nursing skills to assess physical, psychosocial, and emotional needs, respecting patient values and cultural considerations.

Qualifications and Skills

• Licensed RN with a Nursing Diploma/Associate's degree; Bachelor’s preferred.
• No prior experience required; 2+ years of experience preferred for advanced roles.
• Excellent assessment, critical thinking, and communication skills.
• Ability to work safely and maintain alertness in a fast-paced environment.
