Full-Time Clinical Nurse - Transitional Care

Role: Registered Nurse in the Transitional Care Unit at a Level I Trauma Center. Responsibilities Provide direct patient care, including assessment, diagnosis, planning, implementation, and evaluation within nursing standards.Promote patient and family-centered care, educating on illness management, medication, and post-treatment needs.Participate in activities that ensure patient safety, quality, and regulatory compliance; engage in professional development.Develop, implement, and document individualized care plans collaboratively with the healthcare team, patients, and families.Assess physical, psychosocial, and emotional needs, considering patient preferences, values, and cultural factors.Work in a safety-sensitive environment requiring alertness and safe practice. Qualifications & Skills Minimum: Nursing Diploma or Associate's Degree and RN licensure; no prior experience required.Preferred: Bachelor's Degree in Nursing and at least 2 years of experience.
