Clinical Nurse - Transitional Care Unit

Responsibilities

• Provide direct patient care, including assessment, diagnosis, planning, implementation, and evaluation according to nursing standards.
• Collaborate with multidisciplinary team members such as physicians, therapists, case managers, and pharmacists to ensure optimal patient outcomes.
• Educate patients and families on managing illnesses, post‑treatment care, and medication administration.
• Develop, implement, and document individualized care plans, communicating effectively across care teams.
• Promote patient safety, quality, and regulatory compliance, participating in ongoing professional development.
• Assess physical, psychosocial, and emotional needs, considering patient values, cultural, spiritual, and demographic factors.

Qualifications

• Minimum: Nursing Diploma or Associate’s Degree, active RN license; no previous experience required.
• Preferred: Bachelor’s Degree in Nursing and at least 2 years of nursing experience.
• Strong critical thinking, assessment, and clinical decision‑making skills.
• Ability to work in a fast‑paced environment while maintaining safety and quality.
