Clinical Nurse - Transitional Care Unit

About the Role

Provides direct patient care in a fast-paced, multidisciplinary environment within the Transitional Care Unit, focusing on diverse medical conditions. Collaborates with healthcare team members to develop and implement individualized care plans, ensuring patient safety, quality, and regulatory compliance.

Responsibilities

• Assess, diagnose, plan, implement, and evaluate patient care following established standards.
• Educate patients and families on illness management, post-treatment care, and medication administration.
• Promote patient safety and participate in professional development activities.
• Communicate care plans effectively across the continuum of care, considering patient preferences and cultural needs.

Requirements

• RN license required.
• Nursing Diploma or Associate's degree in nursing; Bachelor's degree preferred.
• No prior experience necessary; less than two years preferred.
• Strong assessment, critical thinking, and communication skills.
• Ability to work safely and maintain alertness in a safety-sensitive role.

Schedule

This PRN position involves working two twelve-hour shifts every six weeks.
