Clinical Nurse - Transitional Care Unit

About the role

This position provides direct patient care within a Transitional Care Unit that supports boarding patients in the Emergency Department awaiting inpatient beds.

Responsibilities

• Deliver patient-centered care, including assessments, planning, implementation, and evaluation.
• Educate patients and families on managing illness, medications, and post-treatment care.
• Promote safety, quality, and regulatory compliance; participate in professional development.
• Develop and document individualized care plans collaboratively, ensuring effective communication across care teams.
• Assess physical, psychosocial, emotional, cultural, and spiritual needs, considering patient preferences and barriers.
• Work in a safety-sensitive environment, maintaining constant alertness and safety awareness.

Requirements

• Nursing diploma or associate's degree in nursing; RN licensure required.
• Preferred: Bachelor's degree in nursing and less than 2 years of experience.
• ED and/or inpatient experience is helpful but not mandatory.

Schedule

Night shifts on a 3x12 schedule.
