Registered Nurse in the Transitional Care Unit, providing direct patient care, assessment, planning, implementation, and evaluation within nursing standards.

Responsibilities

• Deliver patient- and family-centered care, educating on illness management, post-treatment care, and medication administration.
• Participate in safety, quality, and compliance activities, and engage in ongoing professional development.
• Develop, implement, and document individualized care plans collaboratively with the healthcare team and patients or caregivers, ensuring effective communication across care settings.
• Utilize critical nursing skills to assess physical, psychosocial, and emotional needs, respecting patient preferences, values, and cultural considerations.
• Collaborate with multidisciplinary teams, promote safety and quality standards, educate patients and families, and develop comprehensive care plans tailored to diverse patient needs.

Qualifications & Skills
