Clinical Nurse - Transitional Care Unit

Role: Registered Nurse in the Transitional Care Unit Responsibilities: Provide direct patient care including assessment, diagnosis, planning, implementation, and evaluation within established nursing standards. Collaborate with a multidisciplinary team to develop and communicate individualized care plans, educate patients and families on managing illnesses, and promote safety and quality compliance. Use critical nursing skills to evaluate physical, psychosocial, and emotional needs, respecting patient values and cultural considerations. Qualifications & Skills: Required: Nursing Diploma/Associate's Degree, active RN license, safety-sensitive position. Preferred: Bachelor's Degree in Nursing and two years of experience. No supervisory experience necessary. Key Duties Care for patients with diverse and complex medical conditions in a fast-paced environment.Participate in professional development and ensure communication across care teams.Support patient safety, quality initiatives, and regulatory standards. Additional Information: This PRN role involves working two twelve-hour shifts every six weeks, offering opportunities for clinical growth and a foundation for specialty nursing careers, including potential progression to Emergency Department.
