Essential Functions
- Aid individuals in finding safe and affordable housing within Erie County
- Purchase household furnishings for the individuals' homes
- Help obtain necessary benefits and manage income
- Collaborate with treatment and service providers, families, and other supports
- Assist in locating and referring individuals as needed
- Keep detailed records in the Cerner Database System and other required paperwork
- Attend team and community-based meetings
Skills
- Proficiency in computer software such as MSWord, Excel, PowerPoint, Desktop Publishing, Outlook, etc.
- Knowledge of Community Health Systems
Education Requirements
- Bachelor’s Degree in Human Services or related field with one year work experience with mental health recipients
- Associate’s Degree with two years’ experience working with mental health recipients
- High School Diploma with four years’ experience working with mental health recipients
Experience
- One to four years work-related experience dependent on education
Qualifications
- Valid driver's license and working motor vehicle
Compensation
$21.00/hr - $25.00/hr
