Housing Case Manager
About the role
The Housing Case Manager is responsible for delivering comprehensive care and support to enhance the well-being of service recipients. This includes providing therapeutic support and skills training both in person and virtually, fostering communication with clinical professionals and the individual's support system as authorized. The role also involves acting as a role model and mentor, coaching service recipients using approved techniques and strategies, and implementing physical intervention when necessary to ensure the safety of individuals.
Responsibilities
- Deliver comprehensive care and support to enhance the well-being of service recipients.
- Provide therapeutic support and skills training both in person and virtually.
- Foster communication with clinical professionals and the individual's support system as authorized.
- Act as a role model and mentor, coaching service recipients using approved techniques and strategies.
- Implement physical intervention when necessary to ensure the safety of individuals.
- Meticulous completion of documentation and reporting for grant programs.
Requirements
- Required Education: Bachelor's degree in health or human services.
- Acceptable Degrees: Applied Behavior Analysis, Child Development, Counseling, Early Childhood Education, Early Childhood Intervention, Education Psychology, Gerontology, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Registered Nurse, Rehabilitation & Rehab Studies, Social Work, Sociology, and Special Education.
- Substitutions: OR bachelor's degree with at least 30 hours of behavioral health coursework.
- Required License(s): Valid driver’s license with an acceptable driving record. Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification.
- Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate.
