Forensic Case Manager - Residential Services

Scope of Position

The Forensic Case Manager provides supportive services to formerly homeless adults diagnosed with serious mental illness, seniors, veterans, medically frail individuals, and those with a forensic history, all residing in apartments.

Essential Position Functions

  • Provide case management, including benefits assistance, help complete housing applications, provide links to primary and behavioral health care in the community, and housing referrals.
  • Avoids identification of strengths, needs, abilities, and goals, and create related Support Plans in the electronic client records system.
  • Motivate clients to develop skills needed to maintain housing.
  • Afford support and encouragement to individuals to develop natural supports and include these supports in Support Plans and client-related meetings.
  • Afford assistance and encouragement to clients in completing their HRA packet in an effort to locate and obtain more independent housing.
  • Ensure timely completion of required documentation for every client contact as per agency policy.
  • Afford compliance with requirements, including facilitating appropriate admissions and discharges, maintaining census, documentation and incident reporting.
  • Liaise with other departments and outside agencies.
  • Work with consumers/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.
  • Conduct apartment inspections on multiple floors, utilizing stairs. Provide escorts to clients throughout the community utilizing public transportation. Provide assistance and instruction to clients in maintaining their home environment in decent living conditions.
  • Immediately report serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors.
  • Complete incident reports in accordance with Bridge policy. Account for consumers, perform diligent searches, and file missing person reports on consumers not accounted for in accordance with policy and procedure.
  • Attend staff meetings, supervision, and trainings as required.

Qualifications

  • Bachelor’s Degree is required.
  • Experience working with mental health, crisis intervention, and managing trauma-triggered behavior.
  • Excellent time management skills; stress-resistant, flexible, and a team player.
  • Open to feedback; strong organizational skills.
  • Willingness to perform CPR/First Aid/AED/CPI/Naloxone.
  • PREFERRED: Preferred experience working with the forensic population.
  • KNOWLEDGE OF AWARDS IS A PLUS.

Hours

Full-Time, 35 hours per week, with a variable schedule that may include weekends.
