Family Care Coordinator / Social Work / Full-Time
Summary of Performance Expectations
Provides counseling and guidance support to patients and families throughout the hospital as needed.
Assesses and evaluates discharge planning needs in collaboration with physicians, direct care nurses, Patient Care Managers/Discharge Planners/Social Worker, and associated clinical professionals.
Participates in interprofessional team meetings.
Affords assistance to patients and families in obtaining financial assistance, psychosocial support, community resources, advance directive education, and medical equipment.
Works closely with community agencies that provide services and resources to patients and families.
Facilitates support groups and collects data required for accreditation programs when applicable.
Required Education & Credentials
- Master’s degree in Human Service Field (Counseling, Sociology, Psychology).
- May consider a Master’s degree with five or more years of experience with mental health counseling.
