Requirements
- Current pharmacist license through the state board where services will be provided:
- West Virginia: West Virginia State Board of Pharmacy
- Pennsylvania: Pennsylvania State Board of Pharmacy
- Certification in Basic Life Support within 60 days of hire date
- State criminal background check and Federal background check (if applicable), as required for regulated areas
Qualifications
Minimum
- Pharmacist license as specified above
Preferred
- Doctor of Pharmacy (PharmD) from an accredited school of pharmacy
- Completion of a residency training program
- Three (3) years of pharmacist experience
