Emergency Department Registered Nurse Care Manager

Role: RN Care Manager in the Emergency Department Responsibilities Coordinate care for diverse ER patients, ensuring safe transitions and optimal outcomes.Perform rapid assessments and develop individualized care plans.Collaborate with physicians and interdisciplinary teams to deliver comprehensive care.Arrange patient admissions and discharges, facilitating smooth transitions.Connect patients with community and social resources to support ongoing needs.Advocate for patients and support families throughout the care process.Promote equity and culturally competent care using evidence-based practices.Contribute to quality, safety, and process improvement initiatives in a fast‑paced environment. Qualifications & Skills Registered Nurse (RN) licensure.Experience in emergency or acute care settings preferred.Strong assessment, care planning, and collaboration skills.Ability to advocate for patients and support diverse populations.Knowledge of community resources and social determinants of health.Commitment to culturally competent, evidence-based care and continuous improvement.
