About the Role

Responsible for managing a home therapy program to deliver quality patient care while maintaining cost-effective operations. Provides leadership, coaching, and development for staff, ensuring compliance with legal, regulatory, and safety standards. Oversees patient care from admission to discharge, ensuring effective treatment, education, and safety for patients with End Stage Renal Disease (ESRD). Collaborates with internal teams and external partners to support program growth and quality goals.

Responsibilities

  • Manage program operations, results, costs, growth, and staffing.
  • Lead, train, and evaluate staff; ensure ongoing compliance with policies and procedures.
  • Coordinate patient care, oversee assessments, treatment plans, and education.
  • Address patient concerns, facilitate care coordination, and ensure safety standards.
  • Implement quality improvement initiatives and support program development.
  • Maintain facility and environmental safety, manage resources, and ensure regulatory compliance.
  • Participate in meetings, strategic planning, and projects related to program growth.
  • Act as an advisor to staff and address patient concerns.

Requirements

  • Graduate of an accredited School of Nursing (RN) with current licensure.
  • Minimum one year of nursing experience, including six months in dialysis.
  • 3+ years of supervisory or project management experience preferred.
  • Proficient in home therapy equipment operation and troubleshooting.
  • CPR certification and ability to pass Ishihara Color Blindness test required.
