Dementia Care Navigator

Position Summary: The Dementia Care Navigator supports patients and families throughout the dementia diagnosis process. Collaborating with physicians and advanced practice providers, they help coordinate medical, behavioral, and social services while providing education, counseling, and resource navigation. They ensure impactful, high-quality, meaningful outcomes and caregiver satisfaction.

  • Supports patients and caregivers in understanding care plans and identifying unmet social determinants of health.
  • Provides emotional support and counseling to caregivers and family members.
  • Works closely with the care team to implement individualized care plans and coordinate referrals to specialists, therapy services, home health, and community programs.
  • Facilitates communication between the care team, patients, caregivers, and outside providers.
  • Maintains awareness of patient and caregiver needs and identifies emerging risks or barriers to care.
  • Assesses risks related to dementia progression, including wandering, medication adherence, and caregiver burnout.
  • Develops safety plans and contingency care strategies for families.
  • Documents all patient interactions and care coordination activities in the EHR.
  • Tracks key metrics related to patient engagement, caregiver support, and resource utilization.
  • Participates in interdisciplinary team meetings and case conferences.

Qualifications

  • Bachelor’s Degree in Social Work, or an equivalent degree in the social &/or behavioral sciences from an accredited program, or a nursing degree with associated licensure.
  • Minimum of two years in behavioral health, eldercare, or care coordination.
  • Preferred experience working with geriatric populations and dementia care.
  • Knowledge of community resources for older adults and caregivers.
