Patient Navigator - Care Management - Days

Essential Job Duties

• Affords patients an understanding of their treatment options and the resources available within Baptist and the surrounding community.
• Facilitates positive relationships with healthcare professionals and businesses to ensure patients receive necessary care and services.
• Trains patients on accessing healthcare services, including roles of Primary Care Physicians, Urgent Care Centers, and Emergency Rooms.
• Empowers patients and families to make autonomous health care decisions and adhere to post-care instructions.
• Links underfunded/uninsured patients to financial counselors or enrollment services.
• Maintains accurate records of patient contacts, referrals, and services provided, and adheres to policies governing the use of this information in the Electronic Health Record.

Requirements

• Bachelor's Degree in Social Work
• 1-3 years of related experience
• No required licenses/certifications
