Crisis Navigator (TAP)

About the role

Crisis Navigator (TAP) in the CPH (Community and Public Health) division of the CSFD (Colorado Springs Fire Department).

Perform assessments, intensive case management, behavior modification, and solution focused treatment for members of our Transitional Assistance Program (TAP).

Responsibilities

• Conduct daily outreach in a team with a Behavioral Health Clinical Navigator for those experiencing homelessness in Colorado Springs.
• Conduct intensive case management and community navigation for members on a daily basis; manage a caseload of high-needs clients.
• Respond to requests from the community for unhoused individuals experiencing behavioral health crisis for assessment and assistance.
• Create intervention planning/goal setting for each member assigned.
• Complete case conceptualization/presentation in case review, internal staffings, and community meetings.
• Represent CPH at community meetings and report outcomes to supervisor.
• Implement behavioral modification/solution focused treatment for issues that are hindering improvement.
• Complete psychoeducation specific to the client's reported diagnoses and symptomology.
• Complete intake/assessment for those individuals referred to CPH programs for additional support.
• Coordinate care with various community partners including, primary care, behavioral health, substance abuse, and other available resources.
• Participate in case consultation, internal and external, to assist compliance of member.
• Interact in professional-to-professional communication to advocate for member.
• Absorb referrals and coordinate care.
• Assist with community outreach, building relationships with partners/agencies, and maintaining positive alliances.
• Maintain monitoring of emergency medical response interactions and notifications for compliance.

Requirements

• A bachelor’s degree from an accredited college or university with major coursework in social work, counseling, psychology, or related field.
• One year of full-time experience working with patients in a behavioral health setting.
• Possess, or obtain upon hire, and maintain a valid, non-probationary Colorado driver's license not subject to restriction related to alcohol and/or drug violations or pending charges.

• Communicate professionally, effectively, and efficiently with all stakeholders.
