About the role

The Crisis Navigator “CN” provides clinical and support services to consumers and/or their families through integrated behavioral healthcare services and resource navigation. The Crisis Navigator “CN” will assist the individual to explore and develop social, occupational, leisure, and living skills, as well as linking to ongoing treatment in the continuum of care. The Resource and Walk-in Crisis Center provides 24/7 support to individuals who are experiencing a mental health crisis or looking for resources to prevent a crisis. This position collaborates with emergency services staff to provide referral and transition services to consumers needing a higher or different level of care.

Responsibilities

  • Provide prevention/early intervention, care navigation, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position.
  • Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider and family members as needed.
  • Provide brief interventions using evidence-based techniques such as Motivational Interviewing, de-escalation, or other treatments appropriate to support consumer engagement.
  • Document initial intervention, track patient follow-up, and clinical outcomes using an Electronic Health Record system (EHR). Document in-person and telephone encounters in the EHR, and use the system to identify and re-engage patients.
  • Attend collaborative treatment team meetings and reviews of consumers
  • Provide therapeutic crisis intervention and emergency services as needed
  • Aid eligible consumers in gaining access to needed medical, social, educational, and other services.
  • Facilitate and contribute to the accomplishment of tasks by consumers, empower consumers to reach optimum level of achievement, and set a positive example and provide role modeling, as well as technical expertise, to accomplish goals.
  • Attend and participate in group sessions, in-service education and training opportunities, meetings, conferences, and special events in order to utilize opportunities for professional growth.
  • Provide transportation for consumers in agency vehicles when clinically indicated.
  • Work closely with outside organizations on resources and care coordination.
  • Use identified agency key performance indicators to track success, engagement, and other factors leading to successful completion of treatment.

Requirements

  • Requires bachelor’s degree in psychology, Social Work, or directly related field
  • 6 months of experience working with adults who have mental illness, particularly those individuals diagnosed with a serious and persistent mental illness preferred but not required.
  • Communication, organization, time management and clinical skills.
  • Ability to work independently in a community-based setting.
  • Valid Colorado Driver’s license with a clean driving record.
  • Bilingual Spanish preferred.

Qualifications

Schedule

Variety of schedules available: Thursday through Saturday 8a-8p, Sunday through Tuesday 7a-7p, or Saturday and Sunday 7a-7p.

Pay

$26.50 per hour + $4-$6/hr shift differential dependent on shifts hired for
Eligible for $250/month Bilingual Stipend
