Salary

$50,000.00 - $52,000.00

Position Summary

The Care Coordinator is responsible for supporting and assisting individuals in COMHAR's Outpatient Programs by coordinating care. This role connects new members to outpatient services through the intake process, provides information and education, links individuals to needed resources, facilitates communication with partner/provider agencies, and encourages members to achieve their recovery goals.

Duties And Responsibilities

  • Provide information, education, encouragement, and care coordination throughout the various phases of recovery, including pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance.
  • Support communication with primary care providers and other medical providers to promote integrated healthcare.
  • Maintain relationships with community partners, acting as a liaison to coordinate care by facilitating communication, obtaining consents, addressing barriers to service, and attending linkage, treatment team, and care coordination meetings as needed.
  • Conduct triage and facilitate the enrollment process for individuals seeking services at the Welcome Center and/or Open Access points.
  • Engage in outreach efforts, including mobile outreach, to connect individuals with appropriate services.
  • Actively identify and offer linkages to community-based resources that support the individual's recovery journey, such as medical providers, recovery communities, educational, vocational, social, cultural, spiritual resources, mutual self-help groups, and professional services.
  • Collaborate with the individual and treatment team (therapists, psychiatric practitioners, nurse supervisors, peer specialists, and case managers) around recovery planning needs related to resources or care coordination.
  • Support individuals in crisis in behavioral health care settings, which may include accompanying them to Crisis Response Centers or delivering necessary documentation to facilitate crisis services.
  • Ensure timely completion of required documentation, including submission of care coordination encounters with individuals, families, or service providers in the electronic health record.
  • Assist Therapists with outreach and caseload management when care coordination is needed.
  • Adhere to a flexible schedule that allows for evening and weekend hours as required to respond to individual needs.
  • Provide support to multiple outpatient programs as needed, including but not limited to Enhanced Outpatient, Specialized Services, and Latino Treatment Program.

Skills

  • Basic understanding of clerical procedures and systems, such as record keeping and filing.

  • Able to provide person-centered, trauma-informed, and strength-based support.
  • Strong organizational skills.
  • Working knowledge of the recovery/treatment system.
  • Flexibility and adaptability in handling changing priorities in a fast-paced environment.
  • Ability to demonstrate professionalism and ethical service at all times.
  • Computer literacy is essential, including effective use of electronic health records, video conferencing, email, Microsoft Office products, and other necessary technologies.

The Enhanced Program is a fast-paced, high-volume, diverse outpatient clinic. On occasion, meetings may take place at other COMHAR sites or in the community. The noise level in the work environment usually ranges from low to moderate.

Requirements

  • Education/Experience:
  • Bachelor's degree in social work, counseling, psychology, sociology, gerontology, counseling education, or a related field.
  • A minimum of 1 year of experience in the health or human service field, with a preference for experience in behavioral health or substance use.
  • Valid PA State Criminal, FBI, Child Abuse Clearances. Clearances must be updated every 3 years per COMHAR policy.
  • Must have a current ACT 31 (Recognizing and Reporting Child Abuse Certificate).
  • Licensed individuals must renew every 2 years.
