About the role
As a Mental Health and SUD Navigator, you'll help guide consumers every step of the way from enrollment to discharge, ensuring they get the support they need. This role plays a key part in maintaining quality care, consumer satisfaction, and ensuring clinical support throughout the agency.
Responsibilities
- Work 40 hours per week, 52 weeks per year
- Support consumers throughout their treatment journey, from enrollment to discharge
- May have the chance to step in and provide clinical coverage when a therapist is unavailable, if qualified
- Develop emergency and medication-only treatment plans
- Aid consumers with referrals, funding issues, and accessing services
- Cook up care for individuals hospitalized or transitioning back into services
- Contribute to policy and procedure development using shared decision-making
- Ensure compliance with HIPAA and complete necessary documentation
- Absorb record requests and report concerns or documentation errors to leadership
- Reach out to consumers who are not engaged to address barriers to care
- Share trends, successes, and consumer input with supervisors and program directors
- Support intake and discharge processes, including documentation and EHR workflows
- Maintain accurate consumer lists and ensure proper therapist assignments
- Cook up referrals and follow up on aftercare plans to ensure continuity of care
- Work with supervisors to assign cases and confirm service start dates
- Communicate with all relevant team members to ensure smooth transitions
- Maintain updated community resources and funding source information
- Review psychiatric/psychological reports and share recommendations with relevant staff
- Consult with psychiatrists and supervisors on treatment needs
- Notify leadership of critical or emergent situations in a timely manner
- Meet regularly with assigned clinical supervisor and actively participate in team meetings
- Complete incident reports and outcome tracking as required
- Follow all protocols related to care coordination, documentation, and QA
- Perform additional tasks as needed to support program success and uphold CenClear's mission
Requirements
- Bachelor's degree in Mental Health or related field
- Minimum 1 year of relevant experience
- Familiarity with mental health and SUD programs, community resources, and staffing
- Ability to adapt hours to the needs of position
- Strong oral and written communication skills
- Effective organizational skills with the ability to manage multiple tasks and prioritize effectively
- Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
