About the role
NFI North is seeking full-time Wraparound Facilitators/Care Coordinators to work in the System of Care Fast Forward program (SOC-FF). This unique position works with an integrated team providing a wide range of therapeutic services to youth and families in New Hampshire.
Responsibilities
- Provide wraparound facilitation and care coordination services to youth and families in need.
- Collaborate with interdisciplinary teams to develop and implement individualized service plans.
- Support youth and families in accessing community resources and services.
- Maintain accurate records and documentation of services provided.
- Travel within the state of New Hampshire as needed.
Requirements
- A Master’s Degree in human services or a related field preferred.
- A Bachelor’s or Associate’s Degree in human services with four years of relevant experience may be considered.
- Experience working with juvenile justice or other at-risk youth populations.
Qualifications
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Knowledge of trauma-informed care principles.
- Proficiency in Microsoft Office Suite.
Skills
- Excellent organizational and time management skills.
- Ability to build and maintain positive relationships with clients and colleagues.
- Strong problem-solving and conflict resolution skills.
- Experience with data entry and record-keeping systems.

Pay
