Community Integrated Case Manager

About the Role

The Community Integrated Case Manager provides case management services in shelter or housing programs and serves as the liaison between the CBI Shelter and Housing Programs and the CBI Patient-Centered Medical Home (PCMH) clinic. This role assists the patient and treatment team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress while in the program. The case manager helps patients access health services, financial assistance, employment, education, social services, and natural supports, as well as develop independent living skills. Responsibilities include assessing patient needs, developing and monitoring individualized treatment plans, and documenting services accurately and timely.

Requirements

• High school diploma or GED and one of the following:
  • 18 credit hours of post-high school education in a field related to behavioral health and two years of full-time behavioral health work experience; or
  • Four years of full-time behavioral health work experience; or
  • An associate’s degree and at least two years of full-time behavioral health work experience; or
  • A bachelor’s degree in a field other than behavioral health and at least one year of full-time behavioral health work experience; or
  • A master’s degree or bachelor’s degree in a field related to behavioral health.
• 1-3 years of experience in a medical setting preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders preferred.
• Current Arizona Driver’s License (valid and in good standing).
• Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101 and CBI Clinical Policies and Procedures.
• Clear 39-month Motor Vehicle Record.
• Arizona Fingerprint Clearance card (program-specific).
