Community Case Manager

What You’ll Do

• Conduct service needs assessments and ongoing client evaluations
• Develop, implement, and review individualized service plans
• Connect clients and families with community resources and support services
• Career with providers, families, schools, medical professionals, and community agencies
• Advocate for clients to obtain needed services and benefits
• Monitor client progress and adjust services as needed
• Aid with discharge planning and aftercare coordination
• Maintain accurate clinical documentation and case notes within required timeframes
• Participate in daily team meetings and case reviews
• Complete chart audits and ensure Medicaid compliance standards are met
• Serve as a liaison between clients, families, referral sources, and providers
• Support clients experiencing psychiatric, substance use, or crisis-related challenges
• Maintain professionalism and compassionate care in all interactions

Qualifications

• Required: Bachelor’s Degree from an accredited college or university in psychology, social work, medicine, nursing, rehabilitation counseling, sociology, human growth and development, gerontology, special education, educational psychology, early childhood education, early childhood intervention, child development therapy, human services, human development, family studies, or a related behavioral health field
• Additional Requirements: Valid Texas driver’s license with acceptable driving history, reliable transportation with proof of insurance, ability to pass criminal background screenings, comfort with providing services in homes and community settings
