Community Integrated Case Manager
Job Summary
The Community Integrated Case Manager provides case management services in shelter or housing programs and serves as the liaison between the CBI Shelter and Housing Programs and the CBI PCMH clinic. The position assists the patient and treatment team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress while in the program. This includes assisting patients in gaining access to health services, financial assistance, employment, education, social services and natural supports as well as developing independent living skills.
Skills/Requirements
- Highschool diploma or GED and either:
- 18 credit hours of post-high school education in a field related to behavioral health and two years of full-time behavioral health work experience;
- four years of full-time behavioral health work experience.
- Has an associate’s degree and at least two years of full-time behavioral health work experience;
- Has a bachelor’s degree in a field other than behavioral health and at least one year of full-time behavioral health work experience;
- Has a master’s degree or bachelor’s degree in a field related to behavioral health.
- One to three years of experience in a medical setting preferred.
- Minimum of six months of recovery from substance use and/or mental health disorders preferred.
- Current AZ Driver's License (valid and in good standing).
- Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101 and CBI Clinical Policies and Procedures.
- Clear 39-month Motor Vehicle Record.
- AZ Fingerprint Clearance card (Program specific).
