Community Case Manager

Pay

The pay for this position is $44,850-$58,908 annually. A $500.00 sign-on bonus is also provided.

Responsibilities

• Works directly with individuals receiving services
• Connects clients with resources
• Documents client progress
• Supports follow-through on treatment plans and referrals

Requirements

• An Associate’s Degree and two years experience or a Bachelor’s Degree
• The ability to establish and maintain supportive relationships with children, youth and families
• Knowledge on how to provide holistic and therapeutic case management services
• Experience working in the mental health field

Qualifications

• Must be willing to relocate to a small town in New Hampshire
