Community Health Navigator

At Reema Health, we believe in a personalized, community-based approach to care. Our Community Health Navigators, or Community Guides, build genuine relationships with members to guide them toward better health.

About the role

As a Community Health Navigator, you'll work remotely and in the field, reaching out to members to enroll them in our care model, build and manage a caseload, and help them achieve their health goals. You'll work closely with the Reema Health team and community resources to support members.

Responsibilities

• Conduct phone and in-person outreach to enroll members in Reema's care model
• Manage a caseload of 80-120 members, covering for other guides when needed
• Assess member needs and build member-centered care plans
• Support members in completing health activities, addressing barriers, and maintaining progress
• Coordinate referrals and support appointments
• Maintain professional boundaries and confidentiality
• Document all member interactions in the Reema App
• Represent Reema with integrity and respect

Requirements

• 1+ years of direct experience with the population/community served, or equivalent bachelor's degree in a relevant field
• Experience with crisis planning, case management, and professional boundaries in community-based work
• Relationship-building, community resource knowledge, and health system navigation
• Comfort working as part of a multidisciplinary team

Qualifications

• 18 years of age or older
• Available to work Monday–Friday during core business hours
• Valid driver's license, insurance, and reliable transportation (if field-based)
• Pass a pre-employment drug screening and background check
• Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA
• Use technology-based tools and systems daily
• Lift and carry up to 25 lbs occasionally
