Community Health Navigator

About the role

At Reema Health, we believe in a personalized, community-based approach to care. Our Community Health Navigators, or Community Guides, build genuine relationships with members to guide them toward better health outcomes.

Responsibilities

• Conduct phone and in-person outreach to enroll members in Reema's care model
• Manage a caseload of 80-120 members, covering for other guides when needed
• Assess member needs and build member-centered care plans
• Support members in achieving their health goals through empathetic guidance and follow-through
• Coordinate referrals and support appointments, including transportation as needed
• Document all member interactions and care plans in the Reema app
• Represent Reema with professionalism and respect, maintaining confidentiality and collaborating with the multidisciplinary team

Requirements

• 1+ years of direct experience with the population/community served, or equivalent bachelor's degree in relevant field
• Experience with crisis planning, case management, and professional boundaries in community-based work
• Relationship-building, community resource knowledge, and health system navigation skills
• Comfort working as part of a multidisciplinary team

Qualifications

• 18 years of age or older
• Available to work Monday–Friday during core business hours
• Valid driver's license, insurance, and reliable transportation (if field-based)
• Passes a pre-employment drug screening and background check
• Complies with federal, state, and local regulations regarding patient confidentiality and HIPAA
• Uses technology-based tools and systems daily
• Lifts and carries up to 25 lbs occasionally
