The CLTS Master Level Support and Service Coordinator provides services to a high intensity caseload of participants and families enrolled in the Children's Long Term Support Program. This includes youth with developmental disabilities and unique behavioral health needs who are at risk of out of home care or who are currently in out of home placements. These youth often require support and services for specific needs that are not met through other service systems that can require enhanced service planning and further approval from DHS.
The goal of the Master Level position is to provide comprehensive assessments, recommendations, and short term interventions related to the resolution or management of family dynamics when the participant has a mental health crisis to assist in prevention of psychiatric hospitalizations and/or out of home placement of the youth. An additional goal of this position is to reduce risk of health and safety of vulnerable youth in the CLTS Program; such as abuse and neglect, by the coordination of necessary services and supports.
The CLTS Master Level Support and Service Coordinator will have frequent and direct contact with participants and families to enhance coping skills and provide assistance in using or obtaining ongoing mental health and other supportive services in the community. Additionally, the CLTS Master Level Support and Service Coordinator will provide immediate direct services as indicated by need until linkage to ongoing care can be established. Most services will be provided to participants in their home and in the community; however, telehealth may be appropriate in certain instances.

Responsibilities
- 60% - Provide direct support services:
- Provide frequent and direct Case Management Services to families with youth with unique behavioral health needs and developmental disabilities who are eligible for CLTS Waiver and CCOP.
- Proactively respond to incidents and work to prevent recurrence of threats to participant health and safety.
- Provide comprehensive assessments of participant and family needs and risks, with a focus on supports to prevent psychiatric hospitalization and out of home placements for youth with mental health needs. This includes assessing participant suicide and homicide risk, mental status, areas of functioning and safety concerns.
- Implement immediate short-term, trauma-informed interventions based on the assessment of participant needs and risks. These services may include but are not limited to: Supportive services, psycho-education to participants and families, facilitation of family meetings and agency staffings, providing referrals to and coordinating services among community agencies, providing transportation to facilitate accessing needed resources, assisting participants with applications for public assistance programs and basic needs obtainment.
- Immediately respond to crises as requested and needed to participants/families that other systems are unable to serve.
- Provide an enhanced level of coordination and communication with treatment teams, other HSD employees, families and collateral agencies.
- Actively participate in appropriate case planning meetings as needed.
- Strengthen connections with Behavioral Health agencies to increase access and improve mental health outcomes for the underserved populations.
- Navigate difficult client relationships and unique family dynamics.
- Develop Behavior Support Plans, Crisis Plans, and CSIS Documents.
- Maintain detailed knowledge of community resources to effectively address client/family needs and help strengthen family network of support.
- Partner with CLTS staff to identify vulnerable clients in need of short term services while being linked to long term services.
- Perform other direct services as assigned.
- 25% - Engage in professional communication through documentation, consultation, and supervision:
- Accurately document all client contacts according to agency and program standards.
- Participate in supervision and collaboration meetings as assigned.
- Provide training and support for other case managers.
- Provide consultation on high-intensity cases.
- Assure for Functional and Financial Eligibility to the programs.
- Document Completed/Updated Functional Screens, Monthly Collateral Contacts, Six Month Reviews, Recertification's, and required face to face contacts.
- Document Critical Incidents and Restrictive Measures.
- Completion of Third Party Administrator (TPA) authorizations.
- 10% - Complete administrative tasks necessary to allow for optimal functioning of the program, department, and agency as a whole:
- Follow all HSD policies/procedures and support program compliance monitoring.
- Attend program or departmental meetings as required.
- Attend trainings as required.
- Perform other administrative duties as assigned.
- 5% - All other duties as assigned.
Qualifications
- Master's degree from an accredited college or university in human services field.
- Current Wisconsin LPC, LCSW, or LMFT, preferred. Training licensing will be considered.
- This position will allow the ability to gain 3,000 supervised clinical hours to work towards clinical licensing.
- Experience with children, youth, and families involved in the child welfare, juvenile justice, mental health, physical disabilities, and/or developmental disabilities systems.
- Ability to navigate difficult client relationships and unique family dynamics.
- Demonstrates strong leadership skills and in-depth knowledge of CLTS.
- Demonstrates familiarity with other Human Services Programs.
- Demonstrates strong teaming skills.
- Must have a minimum of 2 years of Human Services experience and be in good standing with the agency.
- Certified in the Children's Long Term Care Functional Screen.
- Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure 5.02 and 5.27.

Rock County has updated their pay grid in 2023 and it has 14 steps for every classification. Employees are paid every two weeks on Friday (26 checks in a year).
