About the role
Coordinate services for children with physical, developmental, and mental-health needs. Conduct assessments, develop service plans, coordinate service delivery, provide on-going case management, assess risk and protective services, monitor purchased services, and provide client advocacy. Work is performed under the direction of the Children with Special Needs Supervisor in accordance with applicable rules, regulations, guidelines, and laws for the Children’s Long-Term Support Waivers (CLTS).
Responsibilities
- Assess client needs, determine eligibility, apply for and utilize appropriate funding sources.
- Interview client, family, care providers, and others to gather case information.
- Determine client medical, psychosocial, environmental, and financial situation.
- Determine risk and least-restrictive alternatives for care.
- Develop service plans, find and develop resources, access other agencies, make referrals, coordinate paid and non-paid support, and provide advocacy.
- Provide on-going case management by coordinating, monitoring, and adjusting services.
- Monitor purchased services by reviewing service billings and authorizing payments.
- Advocate on behalf of clients; contact family members, friends, medical personnel, and other agencies.
- Document client progress; complete required program forms, obtain medical documentation, and write reports, case notes, correspondence, and other materials.
- Create and present public-awareness information, reports, and presentations regarding services for disabled persons.
- Promote inter-agency cooperation between Trempealeau County Department of Health and Human Services and area hospitals, schools, service providers, and collateral agencies.
- Maintain working knowledge of human-service programs, policies, manuals, guidelines, and statutes.
- Attend and participate in staffing and agency meetings.
- Serve on area or state committees or boards related to social-service programs or service delivery as designated.
- Maintain technical manuals; assist in updating policies and procedures as directed by supervisor.
- Perform general office duties: take and relay messages, photocopy printed material, and other duties as assigned.
Requirements
- Bachelor’s degree in Social Work, Human Services, or related field.
- Two years of case-management experience preferred, or any combination of education and experience that provides equivalent knowledge, skills, and abilities.
- Experience in developing community alternatives for children with disabilities and knowledge of Children’s Long-Term Care, Behavioral Health, and related programs preferred.
- Valid motor-vehicle operator’s license and/or available means of transportation required.
- Social Workers must maintain Wisconsin Social Work licensure or certification to remain in the position; Case Workers may obtain licensure or certification and advance to a Social Worker position.

- Communicate orally and in writing with clients, client families, attorneys, law enforcement, agency personnel, judges, economic-support staff, Social Security, school staff, and medical-care providers.
- Communicate with families using an interpreter or technology.
- Perform basic arithmetic: add, subtract, multiply, divide, calculate percentages, fractions, and decimals.
