CLTS-Children's Long-Term Support-Human Services Professional I-Lead (Training)
About the Role
The CLTS Lead Case Manager supports and promotes the Mission, Vision, and Guiding Principles of the Human Services Department. This position assists supervisors and Support and Service Coordinators (SSCs) serving children functionally eligible for the CLTS Waiver Program, provides temporary "floating" assistance, and delivers training and technical support to CLTS staff. The role involves individualized planning, collaboration with multiple systems, resource development, and ensuring children are in the least restrictive setting possible. The position may also involve cross-training in other children and family program areas, such as Coordinated Services Teams (CST).
Responsibilities
- Provide Comprehensive Training (30%):
- Develop and distribute training materials for CLTS Support and Service Coordinators.
- Provide one-on-one training for new CLTS Support and Service Coordinators.
- Shadow new case managers during home visits and virtual meetings with participants and families.
- Communicate with supervisors regarding the progress of training for new staff.
- Provide Case Management Coverage (25%):
- Cover vacant or leave-of-absence positions to ensure consistent service coordination for children eligible for CLTS Waivers and CCOP.
- Complete/Update Functional Screens, Monthly Collateral Contacts, Six-Month Reviews, Recertifications, and required face-to-face contacts following waiver manual guidelines.
- Conduct assessments for children in out-of-home placements.
- Engage in discharge planning, staff home development, implementation, and day-to-day operations.
- Report critical incidents and provide advocacy and ongoing evaluation of service providers.
- Complete Required Documentation (15%):
- Ensure functional and financial eligibility documentation for programs.
- Document Functional Screens, Monthly Collateral Contacts, Six-Month Reviews, Recertifications, and required face-to-face contacts.
- Document critical incidents and restrictive measures.
- Record all client-related activity in Avatar and complete Third Party Administrator (TPA) authorizations.
- Collaborate with Internal and External Partners (10%):
- Gather, share, and develop resources to ensure coordinated care and health/safety needs are met.
- Participate in supervision, regional meetings, teleconferences, screening, and programmatic meetings.
- Attend team meetings and may participate in Coordinating Committee Meetings.
- Assist in managing waitlists, including sending letters to families, processing CCOP applications, and sharing information with ADRC.
- Provide Backup Support (10%):
- Assist CLTS program staff with mandated program activities as needed.
- Additional Duties (10%):
- Perform other duties as assigned, including special projects related to department and program goals.
Requirements
- Bachelor’s Degree in counseling, social work, psychology, or a human services-related field.
- Social Work Certification preferred.
- Experience with children, youth, and families involved in child welfare, juvenile justice, mental health, physical disabilities, and/or developmental disabilities systems.
- Minimum of 18 months of CLTS experience and in good standing with the agency.
- Certified in the Children’s Long-Term Care Functional Screen with experience conducting Children’s Functional Screens.
- Strong leadership skills and in-depth knowledge of CLTS.
- Current unrestricted driver’s license (not due to corrective lenses) and auto insurance pursuant to Rock County Administrative Policy.

- Knowledge of evidence-based practices, disabilities, court systems, and legal processes related to families.
- Proficiency in Microsoft Word, Excel, and Avatar for documentation and data management.
Working Conditions
- Ability to use a computer, keyboard, copy machine, and fax machine.
- Daily typing of notes and use of computer programs such as Microsoft Word, Excel, and Avatar.
- Work in multiple locations, including the office, clients' homes, and various community settings.
- Potential exposure to physically or verbally abusive clients.
- Physical ability to walk, stand, sit, and be active with children and youth.
Benefits
- Salary: Rock County uses a 14-step pay grid for all classifications. Employees are paid biweekly via direct deposit, with a two-week delay between the pay period and payday.
- Holidays: 12 paid holidays per year, including one floating holiday. Additional holidays may be designated by the County Board.

Rock County is located in south-central Wisconsin along the Illinois border, with a population of 161,188.
