Human Services Professional (Children’s Long Term Support)

About the role

We are growing our Children's Long Term Support team in 2027 and adding two new positions. Under the general direction of the CLTS/Birth to 3 Supervisor, this role assesses and determines both financial and functional initial and ongoing program eligibility, coordinates supports and services on-going to help the child maximize their independence, and provides support to the family.

Responsibilities

• Assess and determine both financial and functional initial and ongoing program eligibility
• Coordinate supports and services on-going to help the child maximize their independence
• Provide support to the family
