Clinical Supervisor: Nursing
About the role

Responsibilities
- Ensure patient care is coordinated and managed appropriately by a team of RN Case Managers and LVNs.
- Serve as the first point of escalation, ensuring support is easily accessible in a timely manner.
- Identify, develop, design, and implement nursing educational programs to support clinical excellence and determine success measures.
- Evaluate the effectiveness of these programs to promote ongoing staff development and patient/customer satisfaction.
- Meet regularly with customers to maintain relationships and provide updates such as readmission reports, offering insights to improve patient outcomes and reduce readmissions.
- Conduct supervisory visits and offer direct support to RN Case Managers in the field.
- Coordinate at least bi-weekly Case Conferences with the clinical team to discuss patients' progress and revise the care plan as needed.
- Review and evaluate each case by reviewing the services provided by clinicians, conferences, record reviews, discussing and verifying impressions, and guiding clinicians to promote more effective performance and delivery of quality home care services.
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care.
Requirements
- Two years' management or supervisory experience within the past five (5) years in a health care setting, preferably in Home Health.
- Graduate of an accredited school of nursing, Bachelor’s Degree preferred.
- Current CA Registered Nurse License (RN).
- Current Basic Life Support (BLS) Card.
- Valid CA Drivers License with acceptable driving record.

- Ability to manage multiple tasks and prioritize effectively.
- Experience with patient care coordination and supervision.

$155,000.00 - $170,000.00 annually, plus a generous annual bonus of $40,000.00, paid quarterly.
Schedule
This role is a hybrid of field work and on-site based at 21HHS office. Office: Spend 2 days per week in the office for administrative and team management duties. Fieldwork: Spend 3 days per week (average 12+ per week) in the field conducting supervisory visits and offering direct support to RN Case Managers.
