Responsibilities

  • Ensure patient care is coordinated and managed appropriately by a team of RN Case Managers and LVNs.
  • Serve as the first point of escalation to support field clinicians in a timely manner.
  • Identify, develop, design, and implement nursing educational programs to support clinical excellence.
  • Evaluate the effectiveness of these programs to promote ongoing staff development and patient satisfaction.
  • Maintain relationships with customers and provide updates such as readmission reports, offering insights to improve patient outcomes and reduce readmissions.
  • Conduct supervisory visits and offer direct support to RN Case Managers in the field.
  • Participate in Case Conferences with the clinical team to discuss patients' progress and revise care plans as needed.
  • Review and evaluate each case, establish immediate and long-term therapeutic goals, prioritize tasks, and develop a plan of care.

Requirements

  • Two years' management or supervisory experience within the past five (5) years in a health care setting, preferably in Home Health.
  • Graduate of an accredited school of nursing, Bachelor’s Degree preferred.
  • Current CA Registered Nurse License (RN).
  • Current Basic Life Support (BLS) Card.
  • Valid CA Drivers License with acceptable driving record.

  • Experience with patient care coordination and supervision.
  • Proficiency in Microsoft Office Suite.

Pay

$155,000.00 - $170,000.00 annually, plus a generous annual bonus of $40,000.00, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.

Schedule

This role is hybrid, combining field work and office-based responsibilities. Office work is conducted 2 days per week, while fieldwork involves 3 days per week (averaging 12+ per week).
