At 21st Century Home Health Services (21HHS), we are a rapidly growing home health agency serving over 4,000 patients in multiple counties in Northern California. Responsibilities
- Ensure coordination and management of patient care by overseeing a team of RN Case Managers and LVNs.
- Serve as the first point of escalation for support, ensuring timely access to resources.
- Identify, develop, and implement nursing educational programs to enhance clinical excellence and staff development.
- Assess staff learning needs and formulate plans for ongoing professional development.
- Maintain regular communication with customers to update them on patient care and offer insights to improve outcomes.
- Conduct bi-weekly Case Conferences to review patient progress and revise care plans as necessary.
- Review and evaluate patient care, assisting clinicians in establishing therapeutic goals and developing care plans.
Requirements
- Two years' management or supervisory experience within the past five (5) years in a health care setting, preferably in Home Health.
- Graduate of an accredited school of nursing, with a Bachelor’s Degree preferred.
- Current CA Registered Nurse License (RN).
- Current Basic Life Support (BLS) Card.
- Valid CA Drivers License with an acceptable driving record.

- Strong interpersonal skills to build and maintain positive relationships with patients, families, and colleagues.
- Excellent communication skills, both verbal and written.
- Proficiency in Microsoft Office Suite and familiarity with electronic health records (EHR) systems.

Schedule
This role is a hybrid position, combining 2 days per week of administrative and team management duties with 3 days per week of fieldwork in the home care setting.
