Clinical Supervisor: Nursing

About the role

Anticipated Coverage areas: Northern Santa Clara County to Hayward/Fremont and Southern Santa Clara County to Santa Cruz.

Responsibilities

• Responsible for in the field oversight of clinicians who provide care and treatment to patients in their place of residence.
• Daily patient activities: Handles incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs.
• Covers training and educational programs for nurses within the field to improve clinical outcomes and competencies.
• Affirms staff learning needs and formulates specific plans and goals for next evaluation period with employees.
• Functions as the first point of contact for field clinicians to assist in patient care escalations etc.
• Coordinates at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes.
• Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Aids clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.

Requirements

• Two years' management or supervisory experience within the past five (5) years in a health care setting, preferably in Home Health.
• Graduate of an accredited school of nursing, Bachelor’s Degree preferred.
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) Card.
• Valid CA Drivers License with acceptable driving record.

Pay

Starting pay is commensurate with relevant experience above the minimum requirements. Wage range: Clinical Supervisor: 143,000.00- 163,000.00 plus a generous annual bonus of $40,000.00, paid quarterly.

Schedule

This is a hybrid of field work and on site based at 21HHS office: San Jose Office: Spend 2 days per week in the office for administrative and team management duties. Fieldwork: Spend 3 days per week (average 12+ per week) in the field conducting supervisory visits and offering direct support to RN Case Managers.
