About the Role
In this regional role, you'll serve as the key connection between hospitals, families, and our Seattle facilities — helping patients transition to post-acute care with confidence and clarity. You'll play a critical role in coordinating seamless admissions, building trust with healthcare partners, and ensuring patients receive the care they need in a setting that supports healing and quality of life.
Responsibilities
- Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers
- Evaluate patient referrals and assist with clinical screenings and pre-admission assessments
- Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care
- Serve as a clinical resource and liaison to families and referral sources
- Collaborate with internal facility teams to align care plans and meet resident needs
- Track referral activity and participate in census development initiatives
Qualifications
- Active LPN or RN license (preferred) or clinical background in healthcare, case management, or marketing
- Experience in skilled nursing, post-acute, or hospital-based discharge planning preferred
- Excellent communication, organization, and relationship-building skills
- Ability to work independently and travel regularly between facilities
- Must reside within commuting distance of Seattle, WA
Pay
Starting salary: $90,000
