Clinical Liaison – RN or LPN Crowne Health Care of Brown Nursing & Rehabilitation Center Alexander City, Alabama Crowne Health Care of Brown Nursing & Rehabilitation Center is currently accepting applications for a Clinical Liaison (RN or LPN) to serve as a key connection between our skilled nursing facility, area hospitals, healthcare providers, patients, and families. This position is ideal for a motivated and relationship-driven nurse who enjoys working with patients and families while helping coordinate a smooth transition from the hospital or other healthcare setting into skilled nursing and rehabilitation care. The Clinical Liaison will work closely with hospital case managers, discharge planners, physicians, facility leadership, and our admissions team to evaluate potential residents, coordinate admissions, and help grow strong referral relationships throughout the community. Benefits Competitive Pay – Pay Scale Based on Experience401(k) with Company MatchingMedical, Dental, Vision, Life & Disability InsuranceFlexible Spending Account (FSA)Paid Time OffHoliday Pay with No Waiting Period Qualifications Current RN or LPN license in the State of AlabamaClinical nursing experience requiredLong-term care, skilled nursing, rehabilitation, admissions, case management, or hospital experience preferredStrong communication and relationship-building skillsExcellent organizational and follow-up abilitiesAbility to review and understand medical records and clinical informationProfessional, dependable, and self-motivatedAbility to travel locally to hospitals, physician offices, and other referral sources as needed Essential Job Functions Respond promptly to patient referrals and admission inquiries.Conduct clinical reviews and pre-screen potential residents to determine appropriateness for skilled nursing and rehabilitation services.Review medical records, diagnoses, medications, therapy needs, and other clinical information prior to admission.Communicate with hospital case managers, discharge planners, physicians, patients, and families regarding placement and available services.Coordinate with facility administration, nursing, therapy, admissions, and other departments to facilitate timely admissions and transfers.Assist with verifying that clinical and payer requirements are met prior to admission.Maintain consistent communication and follow-up with referral sources throughout the admission process.Develop and maintain positive relationships with hospitals, physician offices, case managers, community partners, and other referral sources.Educate referral sources, patients, and families about the skilled nursing, rehabilitation, and long-term care services available through Crowne Health Care.Represent the facility professionally within the healthcare community.Assist with census development and referral growth while ensuring that resident care needs can be safely and appropriately met.Maintain accurate documentation of referrals, admissions activity, and follow-up.
