About The Role
As a Clinical Liaison (LPN/RN) at Aspire Senior Living, you will serve as a liaison between our healthcare team and residents in our Southeast communities, ensuring that their needs are met and exceeded.
- Assessing and developing individualized care plans for residents
- Providing direct care and support to residents with varying levels of need
- Collaborating with physicians and other healthcare professionals to ensure coordination of care
- Communicating effectively with residents, families, and staff to ensure seamless transitions and resolutions
- Participating in quality improvement initiatives to enhance the overall resident experience
Requirements
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure in the state of Missouri
- Experience as a Clinical Liaison or related field setting preferred
- Ability to communicate effectively with residents, families, and staff in our Southeast communities
